1. Use this form to enter the information for review to be eligible for in-service clock hours for your event.
2. Create a committee of the Clock Hour Chair, WAEA Regional Leader, and one other WAEA member to review eligibility.
3. Use this form to verify the committee has reviewed and approved the Clock Hour Offering and it will be reviewed and approved at the next WAEA meeting (based on this form).
